How to Make a Referral

Lindley Habilitation Services accepts referrals for CAP MR/DD clients only in the following counties:

Guilford, Forsyth, Rockingham, Randolph, Alamance, Brunswick, New Hanover, Pender and Wake.  

If you would like to make a referral please have the Case Manager call Mya Williams at (336) 259-8948 and fill out the attached form below then e-mail it to Mya Williams at mya@lindleyhabilitation.com.
