How to Make a Referral
Lindley Habilitation Services accepts referrals for CAP MR/DD clients only in the following counties:
Guilford, Forsyth, Rockingham, Randolph, Alamance, Brunswick, New Hanover, Pender and Wake.
If you would like to make a referral, please have the Case Manager call Kathryn Thill at (336)987-8465, fill out the attached form below then e-mail it to Kathryn Thill at kthill@lindleyhabilitation.com.
